Dr Jacob Cook (Jake) graduated from Swansea University in 2014 in the UK and has worked in Wales and England before moving to Australia. Victoria, Queensland and NSW have all provided a rich diversity of experience in general medicine, including 5 years working in emergency departments, 10 months critical care, 9 months paediatrics, 7 months aged care and a year working exclusively in eyes.

Jake has been bringing these varied experiences into the primary care setting for 3 years and to the Inner West for 18 months. He is a father to 3 children, which has done much to hone his bedside manner with kids, while also robbing him of the interests mentioned in other doctors’ blurbs; he used to enjoy surfing. Regularly attending live music performances and engaging in the arts is how Jake relaxes and engages with the wider community.

Appointment Types

Telehealth Appointments (<10 mins): 1 simple issue e.g. repeat scripts, medical certificates, results and referral letters.

Bulk-billed telehealth consultations are available only to patients who have had a face-to-face consultation with one of our doctors within the past 12 months.

General Appointments (<20 mins): 1 or 2 simple issues 

Long Appointments (20 - 30 mins): multiple or complex isssue/s e.g. mental health care plan, health assessment, minor surgical procedure, implanon insertion and removal, fitness to drive assessment, childhood immunisations and initial work cover claim.

Fees for patients WITHOUT Medicare
